I have been killed by ammo explosion 3 times now with no ammo blow-out panels just death, I have also killed 3, M1A2’s with only Ammo hits and no Blow-out panels again, is this a bug? or is this normal?

This is pure RNG or spaghetti code, every blowout panel has this problem.
It depends on the angle the blow-out rack was struck
If the bulkhead between the ammo rack and crew compartment is breached in any way then the blow-out won’t work, resulting in catastrophic explosion.

As the others have said, depends on the angle the hit came from. Realistic too.
Explosion will expand in the path of least resistance. (That’s the whole principle of the blowout panels, they are deliberate structural weakpoint made to give way easier than everything else in the bustle space).
If the shot went through the bustle’s dividing wall / door, the blast is just going to go through that opening and into the crew compartment. The bustles ‘pressure seal’ so to speak that the panels rely on to keep the crew safe compromised.
